To make the most of your first session, please complete the intake form before arriving to the clinic.
Instructions
- Please download the intake forms that apply to your situation.
- Print the intake forms you downloaded.
- Complete them by hand.
- Bring the filled-in copies to your appointment.
For parents
If you would like to provide us with some information about your child, please complete the parent intake form.
Please note that individuals aged 14 and older must be present for all sessions booked in their name, and that parents cannot attend on their behalf. Therapy sessions will not proceed if you attend such sessions without your child, and you will be responsible for paying an administrative fee equivalent to the full price of the missed session.

After-hours appointments
Due to the secure nature of the buildings, a key card is required to go beyond the main floor of the building before 8 AM and after 5:30 PM. If your appointment is during this restricted access period, please call your psychologist when you arrive and/or wait on the main floor of the building until your appointment time.

Downtown Montreal Location
Driving
The downtown Montreal Clinic is located at 2001 Boulevard Robert-Bourassa, on the 17th floor, at the corner of Maisonneuve Street, above the McGill Metro Station. To access the indoor parking, take Robert-Bourassa Bvld. North and turn right on President Kennedy Avenue. The entrance of the parking lot will be on your right, between Robert-Bourassa Bvld. and Union Avenue.
Public transportation
Take the green line to McGill Station and use the Robert-Bourassa exit. The entrance of the building will be on your left when you exit the McGill metro station. Many bus lines stop at McGill, including lines 15 Sainte-Catherine, 24 Sherbrooke, 61 Wellington, 125 Ontario, 168 Cité-du-havre and the 420 Express from NDG.
Pointe-Claire, West-Island Location
Driving
The Pointe-Claire clinic is located at 6500 Trans-Canada Highway, Suite 400 in Pointe-Claire, south of Highway 40 and East of St-Jean Boulevard.
From Highway 40, take exit 52 toward Saint-Jean Boulevard South. Once on Saint-Jean Boulevard South take the first left onto Holiday Avenue and take the first left again onto rue de La Triade. From the Highway 20, take exit 50 toward Saint-Jean Boulevard North. Drive for about 2 kilometers on Saint-Jean Boulevard North, then turn right onto Holiday Avenue and take the first left onto rue de La Triade. The clinic is located on the 4th floor of the white building at the end of Rue De la Triade.
Public transportation
The clinic is a 2 minute walk from bus routes 200, 201, 202, 204, 485, and a 15-minute walk from the Fairview Pointe Claire shopping mall.
